5 . Jim Thorpe is one of the greatest athletes of all time. He had amazing athletic abilities and was well-known during his lifetime, yet that did not make Thorpe a stranger to adversity.

Thorpe was an American Indian from Oklahoma who developed his extraordinary athletic skills in his youth through hard labor. It was also in his youth that he learned to endure hardship brought upon by racial prejudice. Many would say his childhood was not easy. He grew up poor and at age 9 his twin brother passed away and a few years later he lost both of his parents.

But that did not stop him from doing what he loved and pursuing his dreams. Nothing seemed to stop him, not even stolen shoes. Just hours before Thorpe was going to compete in the 1912 Olympics, somebody stole his shoes. Thorpe improvised (临时拼凑) by getting shoes out of the garbage. The shoes were two different sizes. He wore an extra pair of socks on one foot to even them out.

He still went on to win two gold medals—winning each event he competed in except for one, the javelin (标枪). The javelin was the only event he didn’t win, probably because he had never competed in that event before. It is interesting to note that Thorpe had tried to throw the javelin once before in the Olympic trials. At the time, he didn’t know that he could throw it with a running start. He threw it standing still and was placed second.

At the Olympics,he also took part in the decathlon (十项全能运动). He finished first in two events, third in four events, and fourth in two more. Thorpe ended up finishing third in the world. He was undoubtedly a dominating force that couldn’t be stopped and just kept on going.

In 1989, fresh out of high school, I had the difficult task of choosing a career path before college started in three months. In those days in Pakistan, there were limited choices: becoming a doctor or an engineer, or entering the financial world after getting a business degree. I wasn’t interested in engineering, so that I was left with medicine or business. I couldn’t decide.

My uncle suggested that I do a work placement(实习) to experience it for a month in an international company followed by a month in a hospital. After that, I could make a decision. It seemed like a good idea.

I was accepted for a month’s placement at a foreign bank in Karachi. I got a feel of how the world of finance functioned, made new friends, and generally enjoyed the mostly easy­going work surroundings.

The month passed rapidly, and soon I began working at a leading hospital in Karachi. The experience couldn’t have been more different. The hospital had a stressful environment. The days started early (at 7 am, compared to 9 am at the bank), and were filled with endless duties. And the night calls! This was crazy, working all day, through the night, and again the next day.

I began thinking about my two experiences. The bank had offered a more relaxing atmosphere, better working hours and less stress. The hospital was full of excitement, but studying and training were difficult. It seemed that the business choice was going to win out.

Near the end of my month at the hospital, I was driving home after an especially busy night call. In front of me was a public bus, with college students sitting on the top. As the driver weaved through (穿梭) traffic, I could see the boys shaking from side to side.

A new drug called Lecanemab, however, may be the real deal. Results of a clinical trial, conducted by its makers, Eisai, of Tokyo, and Biogen, of Cambridge Massachusetts, have just been announced in the New England Journal of Medicine(November; 2022). After18 months, it had slowed the progress of symptoms by a quarter.

The trial involved 1795 participants who were in the early stages of the illness. Half received the drug. The others, a placebo(安慰剂). It showed two things. One was the modest but measurable slowing of progression. The other was that an explanation of Alzheimers called the Amyloid Hypothesis(淀粉样蛋白假说) seems correct.

Amyloid is a protein which accumulates in parts of the brains of those with Alzheimers, which is an established sign of the illness. Lecanemab, containing a special antibody, is found to be able to attach itself to amyloid and then attracts immune-system cells to clear the protein away (and measurably did so in those receiving the drug).That suggests amyloid does indeed directly create problems associated with dementia and that Lecanemab can slow down the development of the disease.

This is a small first step. Some experts question whether the test used to show an improvement in symptoms is clinically meaningful because amyloid can be detected only with the help of a piece of expensive equipment, which is not something that can easily be turned into a routine program. Moreover, Lecanemab also caused swelling and bleeding of the brain in a number of participants. Now that the new drug has been shown to work, it can be followed up with further tests.
