Surviving without a Heart

On July 2, 2008, D’Zhana Simmons from South Carolina was given a heart transplant. Doctors at Shanghai Ruijing hospital helped a patient Wu Xiaotian. Wu, 31, has had depression since he was 15 years old. The doctor did an operation on Wu. They put two long lines of electrodes (电极) inside his brain. The 16 electrodes are used to let out electricity on different parts of the brain, which can change the patient’s feelings. Wu can control the “happy switch” through an app on his phone. When feeling down, Wu turns on the switch and the electrodes start working. He is soon cheered up. “It feels like there is power rising from the inside of my body,” Wu said.

The hospital has tested the switch on 26 patients since 2020. Up to now, it has helped cut down a patient’s sad feelings by 60%, said the hospital.

But is the switch safe enough? Although patients like Wu feel great using it, the operation is actually a dangerous one. And doctors are still working to improve it. Also, doctors have to make sure that patients don’t rely on it. Healthy people shouldn’t use it, or it may be like a kind of drug for them.

Dr. Michael Mansour of Massachusetts General Hospital is an early user who’s helped by a form of AI that could someday change the way doctors get information. When a patient comes in with unknown infection (感染), Mansour turns to a computer program called UpToDate. It’s a common tool, with more than 2 million users at 44,000 health care organizations in over 190 countries. Basically, it’s Google for doctors — searching a huge database (数据库) of articles written by experts in the field, who are all getting information from the latest research.

Wolters Kluwer Health, the company that makes UpToDate, is trying to incorporate (整合) AI so that doctors can have more of a conversation with the database.

Some doctors hope to use AI to comb through a patient’s medical history before an appointment (预约). In some cases, Dr. June-Ho Kim, who directs a program on primary care at Ariadne Labs, says AI technology may also help primary care doctors look after patients without the help of specialists. “It will free up specialists’ time to focus on more difficult cases that they need to really home in on, rather than the ones that could be answered through a few questions,” he says.

Dr. Marc Succi, who was a co-worker of Kim, says, “AI will finally prove to be a trusted medical tool. AI won’t replace doctors, but doctors who use AI will replace doctors who do not. It will become very common just like designing a PPT on a computer. It’s that level of leap.”

8 . In early July, FDA granted traditional full approval of Leqembi for the treatment of Alzheimer’s disease (AD). This is the first approved medication proven to slow the progression of AD’s clinical symptoms. A phase three trial among 50-to-90-year-old patients with early AD showed that it can do so by about 27% over an 18-month period. While we cannot extrapolate (推断) beyond 18 months, this represents about a six-month delay in progression of AD symptoms.

A second disease-slowing drug, donanemab, has recently shown similar effects and may get FDA approval by the end of the year.The development of Leqembi is a notable success for medical science in that a disease previously impervious to any disease-modifying medical intervention now has an available, effective treatment. However, paraphrasing Winston Churchill, this is not the beginning of the end but the end of the beginning. The drug does not reverse disease symptoms or prevent progression, it slows progression. And there are considerations that make it inappropriate for many patients with AD. It does, however, lay the groundwork for future AD medications that could alleviate (减轻) suffering from this devastating disease for generations to come.

An important factor for new drugs is their cost. Leqembi’s price tag is $26,500 per year — much more than the current symptomatic medications for AD but comparable or less expensive than new medications for other conditions, for example multiple sclerosis or ALS.

Medicare has announced that it will provide coverage for this medication, although it is requiring that physicians perform certain clinical measures and enter the patient’s data in a registry in order for the drug to be covered, which will increase physician and patient burdens. What’s more, traditional Medicare only covers 80% of outpatient costs, so for those patients without supplemental insurance, a 20% copay for the drug and required MRI scans (核磁共振扫描) might amount to up to $10,000 or more per year out of pocket, but still only a fraction of the cost of a year of in a nursing home.
